Massachusetts Work Permits
- Download the Employment Permit Application for 14 through 17 Year-Olds to fill out. This information helps us fill out the work permit.
First section - completed by employer
Second section- completed by pediatrician
Third section - completed by parent/guardian - During the school year, bring the completed application to the guidance & counseling secretary. If you are under 16, you will need a Certificate of Health from your doctor before the work permit can be processed.
- During the summer, email Robin Dole (robin.dole@norwellschools.org) requesting a permit. Please include in your request the promise of employment from your employer and a copy of either your birth certificate or passport. (If you are under 16, we will also need a Certificate of Health from your doctor, as well as a school record, which we can request on your behalf with the guidance department if you are a Norwell High School student.)
